Rochester General Health System has opened a second center that provides services to help seniors remain in their homes.
The new ElderOne PACE Center is located at 355 North Park Drive.
ElderOne is a part of the Program of All-Inclusive Care for the Elderly that serves people 55 and older who qualify for nursing home care, but are able to live safely in the community. PACE, which exists throughout the country, has been approved by New York for managed long term care. The program is approved by the federal Centers for Medicare & Medicaid Services.
The North Park ElderOne PACE Center provides services that include access to board-certified geriatricians and primary care physicians; physical, occupational and speech therapy; social work; recreational therapy; spiritual groups; and hot meals. Transportation is included. Similar services are also provided at the original ElderONE PACE Center, 2066 Hudson Street, Irondequoit.
